Ice Activities Waiver, Release of Liability & Assumption of Risk

In consideration of being permitted to participate in ice skating, hockey, stick time, goalie training, parties, and any other ice or facility activities (the “Activities”) at the LECOM Event Center, I agree, on behalf of myself and any minors listed on this waiver for whom I am the parent or legal guardian, as follows:

1. Acknowledgment of Risk. Ice skating and hockey are inherently dangerous activities. Risks include, but are not limited to: falls; collisions with other skaters, pucks, sticks, boards, or glass; cuts from skate blades; sprains, fractures, concussions and other head injuries; paralysis; and death. These risks exist regardless of the care taken by the facility and cannot be eliminated.

2. Voluntary Assumption of Risk. I am participating voluntarily. I knowingly and freely assume all risks of the Activities, both known and unknown, including risks arising from the acts or omissions of other participants.

3. Release and Waiver. To the fullest extent permitted by New York law, I release, waive, and discharge the operators of the LECOM Event Center, OYD Live, OYD Management, and their respective owners, officers, employees, agents, contractors, volunteers, sponsors, and affiliates (collectively, the “Released Parties”) from any and all claims, demands, or causes of action arising out of or related to any loss, damage, injury, or death sustained while participating in the Activities or while on facility premises, including claims arising from the ordinary negligence of the Released Parties.

4. Indemnification.I agree to indemnify and hold harmless the Released Parties from any claims, damages, or expenses (including reasonable attorneys' fees) arising from my participation, or the participation of my minor children, in the Activities.

5. Medical Authorization. If I or a listed minor is injured and I am unavailable or unable to consent, I authorize facility staff to secure emergency medical treatment, at my expense.

6. Rules and Equipment. I agree to follow all posted facility rules and staff instructions. Helmets are strongly recommended for all skaters and are required, along with full protective equipment, for all hockey and stick-time participants. The facility may remove any participant for unsafe or unsportsmanlike conduct without refund.

7. Minors. I represent that I am at least 18 years old and that I am the parent or legal guardian of each minor listed on this waiver, with full authority to sign on their behalf. This waiver applies to each listed minor to the same extent it applies to me.

8. Photo / Video. If I check the photo/video consent box, I grant OYD Live permission to use photographs and video taken during the Activities that may include me or my minors, for facility promotion, without compensation. This consent is optional and may be declined.

9. Electronic Signature. I consent to signing this waiver electronically. Typing my full legal name constitutes my signature with the same force and effect as a handwritten signature.

10. Miscellaneous. This waiver is governed by New York law and remains in effect for one year from the date signed, covering all visits during that period, unless revoked in writing. If any portion is held invalid, the remainder continues in full force and effect.
