Participant Agreement, Release of Liability & Life-Saving Medical Authorization

Please read this document carefully. By signing this document, you are waiving certain legal rights, including the right to sue Stemerman Sports Entertainment, LLC dba: LECOM Event Center, its owners, employees, and affiliates.

1. Acknowledgment of Inherent Risks

I hereby acknowledge and agree that participation in activities at LECOM Event Center, including but not limited to ice skating, ice hockey, figure skating, turf soccer, lacrosse, football, field hockey, and general training on both the ice rink and indoor turf field, involves inherent risks of injury. These risks include, but are not limited to:

• Ice Rink Risks: Falls on hard ice, collisions with the boards, contact with hockey pucks, sticks, or skate blades, and severe lacerations or fractures.
• Indoor Turf Risks: Turf burns, trips or slips on turf surfaces, collisions with goals/nets, contact with balls, cleats, or other players, and joint sprains or tears.
• General Risks: Strenuous physical exertion, dehydration, concussions, cardiac events, and exposure to communicable diseases.

I voluntarily assume all risks, both known and unknown, associated with my (or my child’s) participation.

2. Release of Liability and Indemnification

In consideration for being permitted to use the facilities, equipment, and programs of LECOM Event Center, I, on behalf of myself, my heirs, executors, and administrators, hereby release, waive, and forever discharge Stemerman Sports Entertainment, LLC dba: LECOM Event Center, its officers, directors, employees, volunteers, and agents (the “Released Parties”) from any and all liability, claims, demands, or causes of action arising out of negligence, personal injury, property damage, or wrongful death occurring during my presence at or use of the facility.

I agree to indemnify and hold harmless the Released Parties from any claims, actions, or losses, including attorney’s fees, brought by any party arising out of my conduct or participation.

3. Emergency Medical Authorization & Medication Administration

I certify that the participant is in good physical health and capable of participating in strenuous sports. In the event of a medical emergency, I authorize LECOM Event Center staff to secure certified medical treatment or transport if necessary.

A. Epinephrine Administration

• I acknowledge that anaphylaxis is a life-threatening allergic reaction.
• If the participant has a known severe allergy, I agree to provide their own unexpired Epinephrine auto-injector (EpiPen) to be kept on-site during participation.
• In the event that the participant exhibits signs of severe allergic reaction (anaphylaxis), I hereby authorize trained facility staff to administer the participant’s personal Epinephrine injector, or a facility-stocked emergency Epinephrine injector (as permitted by state law), as a life-saving measure.
• I understand that emergency medical services (911) will be called immediately whenever Epinephrine is administered.

B. Opioid Antagonist (Narcan/Naloxone) Administration

• I acknowledge that opioid overdoses are severe medical emergencies requiring immediate intervention.
• In the event that the participant is unresponsive and exhibits symptoms consistent with an opioid overdose, I hereby authorize trained facility staff to administer intranasal Naloxone (Narcan) from the facility’s emergency medical kit.
• I understand that emergency medical services (911) will be called immediately whenever Narcan is administered.

C. Medical Release & Limitation of Liability

I explicitly agree that Stemerman Sports Entertainment, LLC dba: LECOM Event Centerand its staff are acting in good faith to provide emergency first aid under “Good Samaritan” principles. I release and hold harmless the facility and its employees from any liability, adverse reactions, complications, or claims resulting from the administration (or decision not to administer due to lack of recognition or availability) of Epinephrine or Narcan.

4. Rules and Conduct

I agree to abide by all facility rules, safety guidelines, and staff instructions. LECOM Event Center reserves the right to terminate any participant’s session or membership without a refund for unsafe behavior or violation of facility policies.

If the participant is under 18 years of age, this agreement must be completed by their parent or legal guardian, who signs on the minor’s behalf and on their own.
